How to Purchase a Curing Childhood Cancer Tag

1. Visit your county tag office. You will need to bring your vehicle registration information with you.
2. Ask the clerk for a “Commitment to Purchase” form. Because this car tag is not actually in production yet, you must fill out this form to order a tag. Indicate that you wish to purchase the “Curing Childhood Cancer” tag which is currently in its commitment period.  (If necessary, ask for a supervisor to assist you.) If they need further information, the secondary tag type code is 60.

3. At this time, you will pay $50 for the cost of the tag. Although you will continue to use your current car tag, you must save your receipt as proof of purchase. When the tags are available for pickup (after we have reached the 1,000 mark), we will contact you and you must bring this proof of purchase back to the tag office to pick up your new plate.

*We need your help! This tag will not be produced unless we secure 1,000 commitments to purchase prior to July 31, 2004.

*You can purchase the Curing Childhood Cancer tag at any time, regardless of when you are supposed to renew your tag. (For example, if you renewed your plate in September, you may still purchase the Curing Childhood Cancer tag at any time prior to July 31, 2004, because you will continue to use your current tag until the Curing Childhood Cancer tag is produced.)

*If we do not secure 1,000 commitments, Children’s Hospital will issue you a refund in the amount of $41.25 (the amount from each tag purchase that the Curing Childhood Cancer fund receives). We would like to see this tag put into production – be a part of the cure by helping us get 1,000 tag purchases!
